Records include electronic files, paper files or any other format used to document information. You can make requests for health records or corporate information. Everyone treated at one of our facilities has a health record. These records contain documents such as reports from care providers, test results and pharmacy information. You can request records for yourself or a person whose records you have authorization to access. How do you protect my privacy? All of your records are kept strictly confidential.
