Reporting to the Executive Advisor, Aboriginal Health, you will be accountable for providing day-to-day health sector navigation support to Vancouver Coastal Health's (VCH) Aboriginal clients and their families.
Liaising on behalf of clients with medical professionals and other health care providers to ensure their successful access to primary care and community-based health care services, you will be responsible for the facilitation of patient care and the informing of program development, planning and evaluation of services for Aboriginal clients.
Key partnerships in this role are with the Aboriginal Health Team, VCH medical and health care providers and leadership as well as a with a variety of Aboriginal community based agencies.
Key accountabilities include:
- Meeting with Aboriginal clients and their families in the assigned Community of Care (CoC) to assess their support and care needs, their access to appropriate community-based services and to facilitate access to these services through support and education activities, with a strong focus on medical and primary care issues.
- Participating in meetings with VCH care providers to discuss the need of Aboriginal clients and their families to access hospital and community based resources and document the information needs necessary for culturally specific and sensitive care across the continuum of care.
- Planning, developing and implementing Aboriginal community health navigation consultative services to VCH programs and services.
- Making recommendations regarding changes to organizational policies, procedures and practices to the Regional Director.
- Maintaining documentation and a database of client interactions.
- Collaborating with other Aboriginal Health Team members to develop in-house education materials and deliver training programs to staff from all disciplines with respect to the cultural perspective and sensitivities in providing care to Aboriginal clients and their families.
- Providing regular activity reports to the Aboriginal Health Team regarding current issues, trends and outcomes for the designated CoC.
Qualifications and Expertise:
- Bachelor’s Degree in health care, Social Work, Nursing or related discipline and two (2) to three (3) years’ recent, related experience working with Aboriginal communities in British Columbia.
- Preference will be given to candidates of Aboriginal ancestry.
- Valid BC Driver’s License as travel within the health authority is required.
- Extensive knowledge of community-based Aboriginal organizations in British Columbia.
- Extensive knowledge of health issues impacting Aboriginal communities.
- Broad knowledge of Aboriginal and non-Aboriginal services and resources currently available within Vancouver Coastal Health’s (VCH’s) Communities of Care (CoC's) for Aboriginal clients in need of health and social supports.
- Demonstrated ability to deal with cultural issues sensitively.
- Demonstrated ability to initiate and maintain collaborative working relationships.
- Demonstrated ability to communicate effectively both verbally and in writing.
- Demonstrated intermediate skill with operating word processing, spreadsheet, and database programs.
