What is a Community Health Centre? 

Community health centres provide a range of health care services in a single location, including access to public and community health nurses, nurse practitioners, physicians, mental health and addiction counsellors, dental clinics for children, speech therapists, nutritionists, youth drop-in health clinics and more. The Community health centres provide services for people of all ages. The Centre works with community organizations and health care providers, such as your family doctor, to keep people and communities healthy.

JOB SUMMARY:

Works within the context of a client and family centered care model and in accordance with the standards, scope and code of ethics as outlined by the College of Licensed Practical Nurses of BC (CLPNBC) and vision and values of the organization. The LPN practices collaboratively as a member of an interprofessional team within Scope of Practice and Professional/Practice Standards of CLPNBC, and within VCH policies, guidelines and clinical practice documents. The LPN provides care to clients and families based on predictable outcomes and the needs/outcomes can be managed by following an established plan of care or under the direction/supervision/guidance of a Registered Nurse (RN). The LPN works closely with the RN in direct care activities such as assessing, planning, implementing, evaluating and documenting client care. The LPN will support clients and families to achieve and maintain optimal health by performing the following duties and responsibilities.
