Within the context of a resident and family centered care model and in accordance with established vision and values of the organization, works as part of a multidisciplinary team in a residential setting with individuals with severe and persistent psychiatric illness living in a licensed group home. Participates in case planning with clients; interviews clients to identify problems and needs and works with clients to determine appropriate care. Acts as an advocate to assist clients in resolving conflicts and other problems with external organizations. Implements care plans, observes residents behaviour and symptoms, provides feedback regarding resident’s needs, performance and progress. Provides supportive counselling, demonstrations, behavioural management to assist with the development of physical, social, emotional and life skills. In addition, supports the Housing Coordinator to assign/coordinate work to designated Mental Health Workers. Interacts regularly with colleagues and co-workers regarding services and operational activities and communicates with key internal and external stakeholders, committees and task groups as required for conducting daily business and achieving program goals and objectives. Troubleshoots problems and refers unresolved problems to Manager/Housing Coordinator as required.
