JOB SUMMARY:

VANCOUVER COMMUNITY MENTAL HEALTH SERVICES IS COMMITTED TO COLLABORATING WITH CONSUMERS AND FAMILIES TO BUILD ON STRENGTHS, TALENTS AND INTERESTS IN ORDER TO FOSTER A COMMUNITY OF HOPE, GROWTH AND SELF-DETERMINATION.

Within the context of a client and family centered care model and in accordance with established standards of a professional practice, and vision and values of the organization, works as part of a multidisciplinary community team to provide and coordinate treatment services for clients with chronic mental illness, significant substance abuse issues, potentially volatile individuals who have histories of conflicts with the law, social agencies, and limited or no access to services in the community. Provides social, psychosocial, and further assessments, comprehensive treatment plans and services, implements, monitors and adjusts individualized treatment plans as required, and individual counselling, group therapy and education. Establishes and maintains relationships with clients, multidisciplinary team and other community resources. Liaises and consults with court personnel, physicians/psychiatrists, law enforcement and social service agencies and aides client access to services. Participates in committees and meetings, engages in on going team planning and quality improvement activities.
