As Transition Services Manager, you will:
Functional Reporting Relationship
The Manager Transition Services reports directly to the Director, Vancouver Community. Primary working relationships are at the management level across the Vancouver Community (VC) and Vancouver Acute (VA) health services delivery areas as well as Providence Health Care (PHC) managers and include the senior leadership team members, Medical Directors/Managers, Finance Managers, Professional Practice Leaders, Patient Services Managers, Adult and Older Adult Program Managers, Clinical Nurse Specialists (where applicable), nursing and allied health and support staff at all levels, as well as the interdisciplinary team (including physicians), Human Resources, and Information Systems personnel. Reporting directly to the Transition Services Manager are the Transition Coordinators, Program Support Staff, Social Workers and interdisciplinary allied health care staff.
The Transition Services Manager operates within the context of a client service culture to provide organizational leadership with respect to Transition Services management through the assessment, planning, implementation and evaluation of program-based services. The incumbent ensures that the goals and mission are achieved by developing strategic and operational plans, goals and objectives consistent with the approved mission of the health authority. He/She is also responsible for the development, coordination and management of a comprehensive relationship between the Program, Vancouver Community health providers, services, agencies and acute care facilities within Vancouver. The Manager is responsible to oversee, develop and maintain an operating budget of $2.2 Million, and supervise, lead and performance manage a team of thirty to thirty-five (30 to 35) nursing and allied health staff, as well as support staff to ensure that services are provided in the most effective way.
Transition Services provides assessment services, based on the InterQual criteria and other mandated assessment tools, such as InterRAI-HC, for patients/clients in acute care settings who do not meet the Acute or Subacute level of care, but require Alternate Level of Care (ALC). Transition Services staff manage client transition to the community by determining their health needs in collaboration with their families and physicians, and linking each client with appropriate community services, including home care services, residential care facilities, transitional care (Skilled Nursing Facility SNF) facilities, hospital-only alternatives, mental health services, rehabilitation services, etc. As the Transition Services Manager manages clinical resource utilization and reviews program/service delivery to ensure the effective utilization of resources, collaborative and evidence-based decisions, and that client expectations and needs are met, he/she must have the ability to evaluate performance indicators, and collaboratively implement organizational change strategies to correct service gaps.
Guidelines are not always clear, and the incumbent must be knowledgeable of hospital operations, quality/utilization management, and organizational operating plans to be able to make Program and service change decisions. Decisions making is complicated by the need to consult and coordinate action plans with other Community and Acute Program managers in planning and implementing initiatives that have organization-wide implications. The incumbent is required to have initiative, vision and the ability to creatively problem-solve.
Communication and Contacts
The Transition Services Manager consults and liaises with the Director, Vancouver Community as well as colleagues including Patient Services Managers, Adult and Older Adult Managers, Community Health Centre Managers, and other Community Service Provider Managers to coordinate transition services for clients from acute to community settings. He/She also liaises with corporate Finance, Human Resources, and Information Management/Information Services regarding operational and significant issues that require further action.
The Transition Services Manager provides expertise to health care professionals, administrators and external organizations on the programs/services offered, including trends and related issues. Participates in regional and provincial activities related to Transition Services, and as such liaises with Medical Directors, Managers, Patient Services Directors/Managers and colleagues at other health authorities throughout the province and the country as applicable.
