Primary Care professionals at VCH provide primary health care to targeted residents. Clients can come see a healthcare professional about their health care needs and chronic disease conditions. Primary Care objectives are to empower our clients to take an active role in their own care through access to everyday services.

Public Health

Public health professionals are responsible for protecting the health of residents in our communities. For nurses interested in providing public health services for women, infants and children in homes, clinics, drop-in centres, community sites and child care facilities – this is your field!

Staff at the “field level” are assigned to geographic districts and often carry a broad mandate for delivery of health protection programs in those areas. Hours of work vary across health service areas, with the availability of compressed work-week options in some areas.

Home Health

For healthcare professionals seeking the opportunity to develop closer relationships with clients, assume more responsibility for patient care, and work with a variety of clients and health care professionals, Home Health is the answer.

Home Health professionals at VCH meet clients where they are, providing care and support so they can continue to live independently in their own homes for as long as possible. Every day is different with opportunities to exercise critical thinking, design care plans to keep clients healthy & safe, and to help them meet their unique goals.

SUPPORTIVE TEAM ENVIRONMENT

Our Home Care teams are provided independent practice opportunities with professional support from a multi-disciplinary team – allowing them to maintain a sense of community with other caregivers and their multidisciplinary teams. This enables Home Care professional to deal with the challenging aspects of working independently.

HOME HEALTH PROGRAMS:

Mental Health & Substance Use

We offer mental health services in many of our locations. Opportunities for mental health professionals can be found across the spectrum, from community care through to high acuity. Special programs include art therapy, early psychosis intervention, mood disorders, concurrent disorders and peer support. Through our multidisciplinary care teams and partnerships with external agencies, we provide: Diagnosis, treatment, individual and group therapy, rehabilitation, consultation, emergency and urgent services, residential services.
