Community Health Access Centre in Richmond

The Diabetes Education Centre is an outpatient program providing client, their families, and their caregivers with diabetes self-management education and support. Program assistants support our education centre, and our interdisciplinary team includes endocrinologists, registered nurses and registered dietitians.

 Our emphasis is on prevention and chronic disease management with a focus on healthy living. This program provides interactive group education, as well as individual assessment and support. Sessions and appointments include information on nutrition, exercise, blood sugar monitoring and more to support self-management and maximize wellness.

How to access

  • Our services

    Our services include:

    • diabetes group education: 2 classes, 1 week apart (4 hours each);
    • gestational diabetes group education: 1 class (3 hours) and a minimum of 2 follow-up appointments;
    • individual appointments with an endocrinologist, nurse or dietitian educator; follow-up appointments as needed; and
    • insulin starts teaching and support.

    We serve adult clients with Type 1 diabetes, Type 2 diabetes, or diabetes in pregnancy.

    Currently, Richmond Primary Care Network is partnered with Richmond Diabetes Education Centre to provide group pre-diabetes classes in English and Cantonese. Please refer pre-diabetes patients to Richmond Primary Care Network.

  • Get a referral

    A referral is required. Have your care provider complete the Richmond Diabetes Education Centre referral form, attach relevant labwork results and fax them to (604) 244-5571.

    Chinese program available on Fridays in Mandarin and Cantonese.

On the day of your appointment, please register your vehicle using the Validation Kiosk located in the reception area to avoid any parking issues. Signs and instructions are provided at the entrance and within the building.
