Harm reduction: Toward the Heart
Provides community-based assessment and treatment for older adults with dementia-related behaviour problems or complex psychiatric and medical needs requiring multidisciplinary care.
How to access

Check eligibility
Adults aged 65 and over who have developed a mental health problem, or behavioural issues related to advancing dementia, or have medical problems related to aging that complicate the treatment of a mental illness.

Get a referral
Clients must be referral by a health professional. This Richmond MHSU referral form needs to be completed by a physician or nurse practitioner.

On the day of your appointment, please register your vehicle using the Validation Kiosk located in the reception area to avoid any parking issues. Signs and instructions are provided at the entrance and within the building.

This program also provides multidisciplinary assessment and treatment for older adults who have diminishing cognitive function or complex psychiatric and medical needs. Family support and education is also provided.
Services include

- rehabilitation services.
Care is coordinated with primary care physicians, elder care facilities and other community programs. Services are provided on an outreach basis, in the community.
