Who can visit a family health clinic

A family health clinic is by appointment only for registered patients. Family health clinics serve patients who have been matched with a family doctor or nurse practitioner at the clinic by the government’s Health Connect Registry, who live nearby and/or who have family already registered at the clinic.

What to expect

The care team at a family health clinic is the first point-of-contact for ongoing, personalized care close to where patients live. They help patients feel their best—physically, mentally and emotionally.

The family doctor or nurse practitioner is the main person on a patient's family health clinic care team they can talk to about their health. 

Care teams may also include other health providers such as

  • nurses, 
  • social workers, 
  • pharmacists 
  • medical office assistants. 

Depending on care needs, a patient's family doctor or nurse practitioner will coordinate appropriate care with the other health-care providers on the clinic team or outside the clinic. If a patient needs specialized care, teams can also help coordinate referrals to specialists when needed, ensuring comprehensive care.

    If you do not have a primary care provider, register with Health Connect Registry. When a provider in your area becomes available, the team in your community will contact you to get matched.

  • I am a registered patient at a family health clinic and my next appointment is not with my family doctor or nurse practitioner. Why is that?

    Your family doctor or nurse practitioner knows your health care best. Depending on your care need at the time, they will coordinate appropriate care with other health care professionals on the team at the family health clinic or in the community.

  • What if I need specialized care?

    Our team can help coordinate referrals to specialists when needed, ensuring you receive comprehensive care.

  • I am a patient of a family health clinic. How do I book, reschedule or cancel an appointment?
