Primary Care Services

Primary Care Network

Primary Care Networks (PCN) are a new way of working together that organizes different teams of healthcare providers together within a geographical community – this includes nurses, pharmacists, licensed practical nurses, registered nurses, mental health and addictions specialists, social workers, pharmacists, dietitians and/or other health-care professionals. These networks will be made up of participating family physician and primary care offices (also known as Patient Medical Homes or ”PMH”), Primary Care Community Health Centres, Urgent and Primary Care centres , and services delivered by not-for-profit community health partners.

Adopting team-based care approach in PCNs aims to increase clients’ access to primary care services, provide strong, effective links to hospitals and diagnostic service providers.

PCN Team Richmond

Urgent and Primary Care Centres

Urgent and Primary Care Centres (UPCC) will provide care for people with unexpected, non-life-threatening conditions who need medical attention within 12-24 hours but who don’t require a visit to the emergency department – this includes sprains, cuts and wounds, an infection, mild asthma attack and less serious child illness and injury. UPCCs won’t replace the emergency department at the hospital for life-threatening conditions (such as heart attack, stroke, major trauma, acute psychiatric care, etc.). The multidisciplinary health care team will be staffed by general practitioners, nurse practitioners, registered nurses and social workers.

UPCCs are intended to be an additional service in the community, working in concert with all touch points for care to provide appropriate urgent services to patients, when and where they need it.

Community Health Centres

At our Community Health Centres, VCH teams focus specifically on the top five percent of Vancouver’s most complex clients who are often marginalized and have multiple health (clinical and psycho social) conditions which impact their day-to-day living (i.e., functionality). Our clients frequently experience homelessness, mental health and substance use disorders, live in poverty, and have difficulty accessing traditional health care services in a doctor’s office-type setting.

Personalized Support & Stabilization (PSS)

Personalized Support & Stabilization (PSS) is a new community team of interdisciplinary professionals working together to support the frail elderly to safely transition, early on, from hospital settings back to their homes in the community. ​

PSS will provide “reablement” care (short-term care at home to aid recovery after discharge from hospital) to help clients increase independence. Reablement is a care approach that supports individuals to regain/maintain functionality, self-reliance and resume the activities that make up their daily lives. It aims to get clients active and back to enjoying the activities they love as soon as possible.

People are referred by acute care and provided with up to eight weeks of individualized care, uniquely designed to help clients achieve their goals, independence and confidence. Individualized care planning, in partnership with the client, will ensure a holistic and patient-centred approach to care.

Public Health

VCH Public Health works to improve the health and well-being of residents through preventing disease, promoting health, and prolonging life among the population as a whole.

Staff at the “field level” are assigned to geographic districts and often carry a broad mandate for delivery of health protection programs in those areas. Hours of work vary across health service areas, with the availability of compressed work-week options in some areas.

Our Home Care teams are provided independent practice opportunities with professional support from a multi-disciplinary team – allowing them to maintain a sense of community with other caregivers and their multi-disciplinary teams. This enables Home Care professional to deal with the challenging aspects of working independently.

Mental Health & Substance Use

We offer mental health services in many of our locations. Opportunities for mental health professionals can be found across the spectrum, from community care through to high acuity. Special programs include art therapy, early psychosis intervention, mood disorders, concurrent disorders and peer support. Through our multi-disciplinary care teams and partnerships with external agencies, we provide: Diagnosis, treatment, individual and group therapy, rehabilitation, consultation, emergency and urgent services, and long-term care services.
