Regional Home Health Education Program (RHHEP)
The RHHEP is a joint project between the Professional Practice and Clinical Education Team and Home Health at Vancouver Coastal Health (VCH) to support the education of nurses who are new to the Home Health Program.

Upon graduation, you will enter into the role of a community health nurse. The program has attracted many VCH internal staff who are looking for a new challenge or change of routine.
In addition to 100% subsidized education, VCH will cover books and wages while in training. The training program consists of eight weeks of classroom and online training mixed with hands-on, preceptored patient care to consolidate learning. Using a preceptorship model, learners are partnered with an experienced home-health nurse who models best practices in caring for clients in the community.
About the Community Health Nurse Role
Community health nurses provide a range of services and support to clients living in the community with acute, chronic, palliative, or rehabilitation-care needs. This care is provided in a team-based care model as a part of an interdisciplinary team, which includes rehab, case management, dietetics and nursing. Home health clients typically have ongoing health issues and/or complex personal and/or social issues, requiring care planning and coordination with appropriate resources. Clients include frail elderly, persons with disabilities and adults with a range of care needs in areas such as chronic disease management, wound management, IV management, and palliative care.
As a Community Health Nurse, you will:
- Work in partnership with a client and family to establish safe, realistic and reasonable goals and provide nursing care and other services for the client in an interdisciplinary team model.
- Deliver direct care and engage in care management to assist clients in managing their own care and navigating through the various services available within the community setting.
Eligibility
- Current practicing registration as a Registered Nurse or Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One year of recent related clinical experience in a relevant clinical area working with complex clients/patients with chronic health conditions or functional mobility issues.
- Valid BC Driver’s License required as local area travel may require the use of a vehicle or other accepted mode of transportation.
