Community/Home Health

Community health nurses provide a range of services and support to clients living in the community with acute, chronic, palliative, or rehabilitation-care needs. This care is provided in a team-based care model as a part of an interdisciplinary team, which includes rehab, case management, dietetics and nursing. Clients include frail elderly, persons with disabilities and adults with a range of care needs in areas such as chronic disease management, wound management, IV management and palliative care.
