Personalized Support & Stabilization Team Plus (PSS+) at RLLF CHC

When hospital care is needed, your function and independence often change, and some people need more support to regain these. Personalized Support & Stabilization Team Plus (PSS+) is a team that helps you get home from the hospital and focuses on your abilities, goals, and what matters to you to be able to live in your home and community.

How to access

  • Get a referral

    • You and your hospital team identify if PSS+ might be a good fit to support your care needs.
    • You will discuss with your hospital team what is important to you and develop a plan for going home.

  • Get support

    Once home, the PSS+ team will work with you to determine your care plan to recover and resume these important activities.  This will look different for each person and includes the following:

    • Specific goals and ways to achieve them
    • Coordinated schedules
    • Ongoing assessment

    PSS+ is a time-limited transition team that utilizes reablement care.  The PSS+ team will help coordinate your transition to other community programs when stabilized.

  • Saturday:   Closed
  • Sunday:   Closed

Closed on statutory holidays.

Parking & Transportation

If taking a bus, please take #20, #9 or #99 and get off at East Broadway and Commercial Drive. If taking the Sky Train, go to the Expo or Millennium Line to Broadway-Commercial station.

There is underground pay parking directly beneath our building. Access this lot from the lane on the east side of Woodland street or the west side of Commercial Drive. Surrounding street parking is also available.

Robert & Lily Lee Family Community Health Centre

At Robert and Lily Lee Family Community Health Centre, you can get basic health care and learn how to keep yourself healthy. We provide services for people of all ages. The Centre works with community organizations and health care providers, such as your family doctor, to keep people and communities healthy.
