Personalized Support & Stabilization Team Plus (PSS+) is a team that helps you get home from the hospital and focuses on your abilities, goals, and what matters to you to be able to live in your home and community.
What to expect
- You and your hospital team identify if PSS+ might be a good fit to support your care needs.
- You will discuss with your hospital team what is important to you and develop a plan for going home.
- The PSS+ team will support your discharge home and coordinate with your hospital team to ensure a smooth transition home.
- Once you are home, your PSS+ team will work with you to develop a care plan to support you in resuming your daily activities. This will look different for each person and include:
- Supporting you in your home
- Setting specific goals with you and ways to achieve them
- Ongoing assessment and care
- Once you have completed the program, your PSS+ team will, if required, help connect you to other community supports as needed.
