Tertiary services provide special holistic, wellness-based support and treatment for adults with serious mental illness whose needs are not met within secondary services such as hospitals, residential care and community.

How to access

  • Get a referral

Your health care professional will complete a referral form. The Tertiary Mental Health and Substance Use Central Access and Discharge team will review the information. A committee of experts from across the VCH/PHC region will discuss and decide which tertiary program is right for you at this time.

  • We will contact your health-care professional

When a space is available in the designated program, the Tertiary Mental Health and Substance Use Central Access and Discharge team will contact the health-care professional who completed the referral. Plans will be made with you and your family to make your transition to the program as smooth as possible.
