RFAST is an intensive outreach team for families where at least one member of the family is facing severe mental illness and/or substance use issues.
What to expect
The team adopts a family lens and works with all members of family, as long as at least one family member is under 19 years old and facing moderate to severe mental illness and/or substance use issues. The team consists of a social worker, nurse and occupational therapist.
The Richmond Family Access Support Team provides a range of outreach-based services such as:
- coordination and continuity of care navigation,
- advocacy and equity-focused services,
- recovery and psychosocial rehabilitation.
